Remote Athlete Assessment

Welcome to the Remote Athlete Assessment!

Please record the following videos in the order listed on your phone for later upload to the client portal.

1. Lunge w/ Extension

    • Throwing side only
    • Step out into throwing stride length

2.  Neck Flexors

    • No shirt
    • Take video from side, standing

3. Cervical Flexion / Rotation

    • Watch complete video…
    • Record from both the front and the side
    • Keep head straight

4a. Ankle Mobility – Dorsiflexion

    • Watch complete video…
    • Place toe 4 inches from the wall

4b. Ankle Mobility – Inversion / Eversion

    • Make sure to film while seated similar to the video
    • Keep knee and leg locked

5. Shoulder ROM (180 degrees)

    • No shirt
    • Record from the back and the side
    • Straight arms O/H,  no shrugging

6. Split Stance Standing Shoulder IR

    • Keep shoulder at 90 degrees and elbow still through out
    • Record from both sides, left and right arm (both sides)
    • Keep neutral wrist throughout

7. Split Stance Standing Shoulder ER

    • Keep shoulder at 90 degrees and elbow still through out
    • Record from both sides, left and right arm (both sides)
    • Keep neutral wrist throughout

8. Forearm Pronation / Supination

    • Keep elbow and shoulder at 90 degrees
    • Throwing arm ONLY

9. Wrist Flexion / Extension

      • Keep elbow straight
      • Record both arms, left and right side

10. Reach Behind Test 

    • No shirt
    • Watch complete video…
    • Record on both sides
    • Try to touch finger tips together

11. Separation Test (hitters only)

    • Keep hips still through out
    • Create as much space as possible with arms and hands
    • Hitters ONLY

12. Thomas Test

    • Watch complete video…
    • Keep leg relaxed while lowering
    • Record from side and front, both left and right legs

13. Prone Hip ER

    • Record both left and right legs from rear
    • Keep legs at 90 degrees throughout

14. Prone Hip IR

    • You can perform both left and right leg test at the same time, similar to video
    • Keep legs at 90 degrees throughout

15. Faber Test

    • Rest ankle on opposite knee
    • Perform test on both sides, left and right leg

16. Lumbar Locked Rotation

    • Sit back on heels
    • Perform test on both sides, left and right side
    • Do not lean away from the rotating side

17. Lumbo-Pelvic Dissociation

    • No shirt
    • Keep upper body still

18. Single Leg Squat

    • Use a chair
    • Perform on both sides
    • Keep back straight

19. OH Squat

    • Make sure to video from the side

20. Single Leg Stability (SLDL)

    • Record from the side and the front
    • Both sides
    • Maintain soft knee through out
    • Get torso to be parallel with the floor

21. Ligament Tightness Test

    • Watch complete video
    • Perform all 3 tests on the video
      1. Pull fingers as far as possible
      2. Fully extend elbow
      3. Lockout knees and touch palm to floor

22. Back Squat (5 RM)

    • Watch complete video
    • 5 RM only
    • If you’ve previously recorded your lift following the sample video, please make sure you have access to it for uploads, otherwise please record again
    • We need to confirm that your listed 5RM is performed with proper form

23. Trap Bar Deadlift (5 RM)
